Caremark.com is owned and operated by Caremark, LLC (“Caremark”). The majority of the
Caremark.com website is accessible only to registered users. To become a registered user, you
must receive prescription benefits from a company who has hired Caremark to provide you with
online prescription services. Your employer or Caremark will provide you with the information
you need in order to complete the registration process. There are several areas at Caremark.com
that are open to the general public; however, these areas of the site may provide additional
information or services for registered users.
We have provided this notice to describe the online information collection and use practices at
Caremark.com. This document describes the choices you can make about the way your information is
collected and used. This privacy policy applies only to the information collected on
Caremark.com, and does not apply to information that Caremark collects about you on other
Caremark owned, operated, or sponsored websites or the websites of Caremark affiliates, or in
other forums, including in email correspondences or over the telephone. This policy does not
apply to the “Client Services” section of Caremark.com, which is available only to
Caremark.com clients, and is governed by separate agreements in place between Caremark and its
clients.
In some circumstances, Caremark’s use of your information will also be subject to the
requirements of the Health Information Portability and Accountability Act (frequently referred
to as “HIPAA”). This will be the case when you submit information to Caremark as
your pharmacy (e.g. to request a prescription refill), in which case the terms of Caremark’s HIPAA Notice of Privacy
Practices will apply, or when you submit information to Caremark when it is acting on
behalf of your health plan (e.g. when you complete a health assessment online for a health
program offered by your health plan), in which case your health plan’s HIPAA Notice of
Privacy Practices will apply. If you have questions about which policy applies to information

From time to time, our internal processes may change, or we may offer new features at
Caremark.com. If appropriate, we will revise this privacy policy. We encourage you to return to
this area to read the most recent version of the privacy policy. The information we provide you
on Caremark.com will be treated in accordance with the privacy policy in effect as of the date
you provide information to us. If we alter our practices in a manner that will affect the
treatment of information that you have already provided to us, we will send you a notice of our
new practices. If you do not agree with the privacy practices described in this policy, you
should not provide us with further information. This version of the privacy policy is effective
as of May 1, 2008. Please note that if your information is also protected by HIPAA and so also
subject to Caremark’s or your health plan’s Notice of Privacy Practices, then the
more restrictive of this policy and that Notice will apply to describe what uses and disclosures
we may make of any personally identifiable information you provide to us.
Information Collection
Caremark collects personally identifiable information at Caremark.com from web visitors who are
interested in receiving services it offers, such as prescription benefit, disease management and
specialty pharmacy services, email alerts, newsletters, and event notifications. Caremark also
collects additional information from registered users, such as shipping address, billing
address, credit card number and other information necessary to process your request. In order to
provide you with online pharmacy services, Caremark receives certain personally identifiable
information about registered users from the user’s employer or health plan. If you have
questions about the information your employer or health plan may have shared with us, please
contact your employer’s human resources department or your health plan.
Caremark works with third-party providers to provide specific services offered on Caremark.com,
and may collect personal information as part of the provision of those services. For example,
BrassRing operates the “Careers at Caremark” feature at the “About
Caremark” area of our site, Yesmail sends several of our email alerts, and Shareholder.com
operates the “Investor Relations” feature at the “About Caremark” area
of our site.
A Note About Cookies and Passive Tracking Technologies: A cookie is a piece of data stored on
your computer that allows Caremark.com to remember that it has previously communicated with your
browser. Caremark uses both session ID cookies and persistent cookies. For the session ID
cookie, once a user closes his or her browser, the cookie simply terminates. A persistent cookie
is a small text file stored on your hard drive for an extended period of time. Once a persistent
cookie is saved on a user's computer, only Caremark.com can read it and no other website.
Persistent cookies can be removed by following Internet browser help file directions. Like most
standard website servers, Caremark uses log files to collect non-personally identifiable
information. This information includes Internet Protocol (IP) addresses, browser type, Internet
Service Provider (ISP), referring/exit pages, platform type, date/time stamp and number of
clicks.
Information Use
Information you provide is used in order to respond to your request. For example, to fulfill your
order or contact you with information about your order, to send you email alerts, to send you
documents you request, to send you newsletters, or to process your online job application. For
potential registered users, we will compare information you provide to the database of
information provided by your employer or health plan in order to confirm your eligibility to
become a registered user. From time to time Caremark requests information from users via surveys
on Caremark.com. Participation in these surveys is voluntary, and information collected as part
of those surveys is used in compliance with this privacy policy.
We may use the information to provide you with materials about Caremark or its affiliates or
Caremark or our affiliates’ products and services, including new content or services at
Caremark.com. We may provide you with these materials by phone, mail, facsimile, or email. We
may also share your information with your health plan, our subsidiaries, affiliates, or business
partners.
We will disclose your personal information if, in good faith, we believe we are required by law
to do so, including but not limited to disclosure to law enforcement or other government
officials in connection with an investigation of fraud, intellectual property infringement, or
other activity that is illegal or may expose us to legal liability. We may also disclose your
information in the event of a merger, acquisition, reorganization, bankruptcy, or other similar
event. If this occurs, your personally identifiable information may be transferred to our
successor or assign, or may be sold as an asset of our company. This does not apply to
information we hold on behalf of your health plan which, as stated above, may be used and
disclosed only for those purposes permitted by your health plan’s Notice of Privacy
Practices. Passive tracking mechanisms such as cookies and log files are used by Caremark to
analyze trends, administer the site, track users’ movement and gather broad demographic
information for aggregate use. Persistent cookies enable Caremark to track and target the
aggregate interests of Caremark.com users to enhance the experience on the site.
To register on this site, we collect Personally Identifiable Information, or PII. This includes
information like your name, address, email address and/or birth date. This PII is maintained
throughout the time you are an active CVS Caremark member. If your membership status changes, we
maintain this information securely in accordance with our Records Retention Policy and
Procedure. Our current policy is to retain this information for seven (7) years after your
membership status terminates.
Contacting Caremark and Options
At the time information is collected, you are provided with the ability to opt out of having
Caremark email you information about Caremark or its products and services, to the extent
that sending such materials are unrelated to the purpose for which you provided your
information. Registered users who wish to alter the uses Caremark makes of their information
may do so at the My Account area of Caremark.com. You may also contact us to exercise

registered users may update their profiles at the My Account area of the site. You may also update your information by

Security
Caremark takes reasonable technical and procedural precautions to protect the information
provided by users of Caremark.com. When online registration and order forms require users to
provide certain information, such as a credit card number, that information is collected on a
“secure” page, and the information submitted is encrypted. To determine if you are
on a secure page, look to see if a lock icon appears on the bottom of your website browser.
